Symptom narrative

Hepatitis B vaccine given at 0831, pt tolerated well. Received call from mother of pt at 0916 stating pt was having severe pain just above right shoulder blade. Pain with deep breaths. Some tingling down the right arm. Pt returned to office for assessment. Treated with ice and ibuprofen.
