Chief Complaint Patient presents with ? Arm Complaint Left arm pain from the vaccines x yesterday. nb ? Syncope Pt fainted yesterday @ 10pm. nb Ibuprofen 2 last night, 2 today Arm pain last night, arm was tingly, then whole body went tingly, then passed out, fell onto stone fireplace Hit back on fireplace ledge, body was shaking Mom thought also hit head but no headache or pain Happened around 10 pm last night Took a minute for her to wake up, mom called 911, was checked out by paramedics, not transported to hospital Was really hot after but talking right away No post ictal phase Back sore today- whole left side but arm better Hadn't eaten much yesterday Did have seizures in past but none in 10 years Will submit VAERS report due to episode and need for medical follow up but would still get Men B #2 in 6 mo (would make sure she eats and drinks well day of visit) Ibuprofen prn pain Local wound care for abrasion disc, no signs of infection at this time Reck if not improving, if symptoms change, worsen or are not responding to treatment plan as discussed
