Clients record showed no history of Hepatitis A vaccine, so per school age required vaccines, administered a Hep A vaccine. Days later, mom noticed that a record she had from another facility showed a Hepatitis A vaccine was given, which made the dose on 7/2/25 invalid/unnecessary.
