Symptom narrative

When reconciling inventory--I discovered that the count of diluent versus vaccine did not match. After some investigation, the logical conclusion of this is that the YF-VAX was given with the sterile water diluent instead of the saline provided with YF-VAX. Discussed with manufacturer. The administering staff member has been made aware of the error and attempt to contact the patient has been made to get them to return and repeat the dose. Have still not heard back at this time.
