Symptom narrative

Patient at Health center to have injection site checked. Had Pneumococcal vaccine on Friday 7/11/25. Bruising noted at left arm injection site with large area of erythema surrounding left arm. Dr. brought to room to examine post injection site. Patient had rubbed arm immediately following injection on Friday, with small purple area noted at injection site on day of vaccine approximately 10mm. Patient stated he had chills Friday night and muscle aches Saturday. Bruising of left arm spread on Saturday across upper left arm with redness surrounding. Dr. advised ice to area 20 minutes on three times daily until redness improved. Patient instructed to return to clinic if not improved in 48 hours. Patient verbalized understanding.

Verapamil SR 180mg , Losartan-HCTZ 100-25mg, Vitamin D3, Omeprazole 20mg, Aspirin 81mg.
