Symptom narrative

The patient was given the HPV vaccine vs the HIb vaccine. This was administered by a medical assistant on 12/2/24 who placed the orders under a provider who was not present in the building at that time. Somehow the provider did not see the HPV order the following day but the order was nonetheless signed 24 hours later. The mistake was not realized until he was seen for routine well child check on 7/3/25. He has been growing and developing normally although the week before, he developed a rash to his left forearm that looks concerning for possible warts but unclear if 2/2 HPV vaccine from DEC or incidental infection.
