Symptom narrative

Client received a vaccine that is expired on 7/13/2025 on 7/14/2025. No signs or symptoms present in client, when they left the facility. Registered nurse had the wrong expiration date noted in her planner and failed to check the vial prior to administration of vaccine. RN called the client's guardian and informed them of what happened and asked if they had any further questions. Also informed them RN had a call out to the health officer for further information.
