Symptom narrative

Patient was given in office for 6 month well visit. Patient was given Hep B Adult 1ml vs the ordered Hep B Pediatric 0.5ml dose in error. As soon as staff realized the error she informed the provider and clinic supervisor. We reported immediately to the parents and completed an internal accident/incident report. Mom will monitor and report any infant fever, increased fussiness and/or redness and swelling to injection sight.
