Symptom narrative

Patient unknowingly received 2 Arexvy vaccines (1 on 12/5/23, and another on 7/14/25). The first vaccine did not show up in the vaccine registry . In addition, the patient did not remember receiving the first dose. She is fine and has not experienced any side effects after receiving the second dose, as per our conversation on 7/15/25.
