Symptom narrative

Using database, pulled a 2 month template that pulled Hepatitis A vaccine instead of Hepatitis B vaccine. MA verified with LVN who also did not notice Hep A vaccine, Hep A vaccine was given at 2 month WCC instead of Hepatitis B. Baby doing well this morning without adverse effects. Family informed and all questions answered
