Symptom narrative

Patient received pneumonia vaccine but had severe arm pain and could not lift objects. Did not resolve in 2 days so patient went to urgent care and received a steroid prescription. Patient improving but when presented to pharmacy, pharmacist noted the vaccine was administered higher than recommended.
