Symptom narrative

Patient presented to office for a well-child visit. Patient was finished with exam and visit. Patient was leaving to go to check out when she stated that she does not feel so good and the clinician that had given patient her vaccine had quickly ran over the patient and lowered her to the floor. Patient did not hit her head but was unresponsive for a period of time. 911 was called and presented to office to take patient to ED to be evaluated. Patient did start responding to the provider.
