Symptom narrative

Vaccine was completed by technician, the correct product was taken out of fridge before vaccine was given to the patient. After the vaccine was given, the technician came to the pharmacist and saw that he had mistyped the expiration date in database and had given an expired product. Patient was left a voicemail to call us back. Patient called back and was told he was given an expired vaccine. He was notified that it was recommended that he could talk with his provider about repeating the dose, or the recommendation is to repeat the dose. He said he would contact his provider.
