This case pertains to one patient who received improperly stored PROQUAD on 06/10/2025; Information has been received from a nurse and referred to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 10-Jun-2025, the patient vaccinated with improperly stored Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) injection (lot #Y019171, expiration date: 18-May-2026, 0.5 mL, frequency, strength, and route were unknown) for prophylaxis (Product storage error). The vaccine was reconstituted with sterile diluent (MERCK STERILE DILUENT) (lot # and expiration date were unknown). No symptom was reported. No additional event was reported.
