Symptom narrative

Set up vaccinations for both patient and brother and another nurse verified them. This nurse was unable to help administer vaccinations so asked me to help. I did not reverify vaccines. Gave me the shots and told me which vaccines went to each child. Vaccines were administered. Once we left room realized we gave the wrong vaccinations to the wrong child.
