Symptom narrative

Site: Swelling at Injection Site-Medium, Systemic: Allergic: Itch Generalized-Medium, Additional Details: Patient came to the Pharmacy on Monday July 7th, 2025 to get shingle vaccine (right arm) and prevnar 20 (left eye). Two days later on July 9th, 2025 patient has itching, swelling, & inflammation on right arm. Patient came to the pharmacy on July 15th, 2025 to pick up Augmentin 875mg from her family doctor for infection. Today, July 16th, 2025 Called & check patient. She said she is getting better.
