Symptom narrative

Five days after injection patient noted large area of "yellowish" skin surrounding the injection site. The yellowish discolored area was sounded by a thin red line. This was noted on the lateral right shoulder and measured approximately 6" x 4 ". The area was slightly itchy and tender but not warm to the touch.
