Symptom narrative

Pt received Yellow Fever on 4/15/25. Pt called facility on 7/29/25 to report that she feels she is having an adverse reaction to the Yellow fever Vaccine. The patient states she began to experience extreme fatigue in June. Pt states she has been seen by her PCP and had labs completed by PCP, states all labs are WNL. Pt states that the fatigue is unrelenting and plans to follow up again with her PCP for further evaluation.
