On 25-JUN-2025, the patient was vaccinated with dose of improperly stored measles, mumps, rubella and varicella (Oka-Merck) virus vaccine live (PROQUAD) (lot # Z003309, expiration date was not provided, but upon internal validation was established as 21-JUL-2026) (0.5mL; dose number, strength, formulation, route and anatomical site were unknown) for prophylaxis, which was reconstituted with sterile diluent (BAXTER STERILE DILUENT) (indication, expiration date, and lot # were not reported) (product storage error).
