Symptom narrative

Irregular menstrual bleeding that began 7/22/25 and has persisted at the time of this form's completion. Saturates approximately 2 pads/day. Migraine for 48 hours began 7/23/25. Cramping and bloating 7/23/25-7/26/25.

Current illness

Pap smear on 7/21/25, before injection, results showed bacterial vaginosis. I had no symptoms (discharge, odor, pain, etc.) before diagnosis.

Medical history

Migraines without aura related to menstrual cycle

Other medications

Estarylla, Vitamin B12, Vitamin D, and Folic Acid daily. Rizatriptan prn for migraines.
