Symptom narrative

Nurse was to administer Tdap vaccine, per provider order. Pulled and administered DTap vaccine, and then realized she had given the incorrect vaccine upon documenting vaccine. Provider and parent was made aware. Call made to GSK to notify and request recommendation for further administration of tetanus vaccine.
