Pt was nervous for vaccines after giving vaccines patient lend back of the chair that's when we gave the patients water took vitals, the vitals was stable patient stayed for 15 minutes for evaluation and was fine and left. The following vital sign readings: at 12:20 pm BP: 96/75 - HR: 69 ' SAT: 94%. At 12:35 pm : BP: 100/79 - HR: 82' - SAT: 99% Also notice the provider about above information.
