pt received Men B injection on 07/29/2025 instead of MCV4 for childhood vaccines. pt came in today 07/30/2025 for reaction on right arm and back red rash. pt denied SOB/chest pain fevers or fatigue. Advised supportive care: acetaminophen or NSAIDs for discomfort, cold compress to the area, and observation over the next 48 72 hours. Instructed to monitor for any signs of progression.
