Symptom narrative

Red to pink, flat, blanching rash to scalp, upper back/shoulders, and few small spots to extremities occurring 7 days after vaccination, rash fading to scalp on day 8. Family attempted use of Zyrtec, oral benadryl, and hydrocortisone cream with no improvement to rash.

Born at 25 4/7 wga, twin, history of retinopathy of prematurity with laser treatment, PFO/PDA, anemia, Bronchopulmonary dysplasia, and history of pulmonary hypertension
