Patient was due for a Hepatitis A on 7/24/25. Instead of receiving Hepatitis A the patient was administered pediarix. Spoke with medical assistant and further training will be done. Spoke with provider regarding vaccine error and if there is any side effects that mother of child should be informed of. The provider stated no side effects. Called and spoke to mother of child and informed her of vaccine error and provider message of no side effects. Asked mother how the patient has been doing and feeling since the vaccine on 7/24/25, mother stated that patient/child has been fine and did not see any change in health. Mother was advised to return to the clinic so Hepatitis A could be administered and child could stay up to date on vaccines. Mother verbalized understanding.
