Reported Symptoms: 10064366:EXTRA DOSE ADMINISTERED; Narrative: From authority: Order was placed during PCP visit for patient to get a second RSV vaccine. Pharmacy found first administration on the immunizations tab. I immediately discontinued the clinic order (15:17). I reached out to the prescriber (15:18). I began a note recommending only 1 dose, current guidelines. Before I could sign the note, nursing had administered a second dose. Provider was made aware of second dose given.
