Reported Symptoms: 10063978: WRONG VACCINE ADMINISTERED; Narrative: From Center: Patient in clinic for routine PCP appt. During intake, patient requested influenza vaccine-in reminders. Patient 60 but accidentally given high dose >65 vaccine. PCP and Pharmacist notified. Other Relevant HX: Other: Incorrect vaccine administered.
