Symptom narrative

Pt was seen at the office on 7/29/25 with his father. Pt had the Tdap vaccine on 7/18/25 at 1133 AM. On 7/21/25 some time in the AM, pt and father report that pt's left shoulder and arm were red and swollen. They did not come in right a way as they were on vacation. Pt and father report that the redness and swelling went on to the other arm and then down to his legs. During all this, pt states he never had any respiratory difficulties; no lips swelling, no report of throat tightening up etc. Pt's father reported that the swelling and redness somewhat went down on the left arm before spreading to the other parts of pt's body. During all this, pt was still actively playing at trampoline park.
