Symptom narrative

Pt came in to get caught up on vaccines and he is 6 yrs old. Mom preferred he get a combo vaccines rather than 4 shots. The box for Pediarix says for up to 6 yrs old and when I chose it in the computer system it gave no warnings or anything when entered and saved. Then I tried to print the Tenniis record for the school after the vaccines were given and it will not verify.
