Symptom narrative

pt says a few days after getting second dose she developed seizures. Sometimes she has partial seizures and other times she has a regular seizure. She will go down and hit the ground and injures herself during these episodes. She now has a service dog. She states they have found odd growths in her body and her memory is being affected as well. She has been ER and ER and hospital. Her initial medical records will be located at hospital. She is being treated by a neurologist now. She has been diagnosed temporal lobe seizures/epilepsy.
