Symptom narrative

Ringing in ears and briefly lost consiousness (LOC) after MCV4 and Men B administered. LOC lasted no more than 5 seconds and patient was being laid down. Symptoms resolved and HPV was given ring of the ears began again but no LOC. Pt felt dizzy when sitting up initially but gradually resolved and was able to leave 30 minutes post injection after given cold water and peanut butter crackers
