Symptom narrative

After about an hour and half after receiving vaccine resident developed rash and edema to injection site , shortness of breath, chills, and itching.

Current illness

Cellulitis to right foot.

Medical history

Polyneuropathy, atrial fibrillation, hypothyroidism, edema, GERD, exocrine Pancreatic Deficiency, Anxiety, and depression.

Other medications

Omeprazole, Acetaminophen

Ciprofloxacin, Succinylcholine, Tikosyn, Amiodarone
