Symptom narrative

Nurse Practitioner calling to report inadvertently administering a PROQUAD vaccine after experiencing a temperature excursion. Product was supported. See TE Case# 02848877. Permission to contact HCP was granted. Additional AE Case# 02848925 filed. No; No adverse event; This spontaneous report was received from a nurse practitioner and refers to a patient of unknown age and gender. The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On an unknown date, the patient was inadvertently vaccinated with a dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) vaccine, (lot #Y013577, expiration date: 03-Feb-2026) for prophylaxis, that on an unknown dates underwent temperature excursions of 17F for 1 Hour 30 Minutes, 17F for 1 Hour 45 Minutes, and 15F for 30 minutes.
