Symptom narrative

Pt was given vaccine by RN and after the pt stated that her arm hurt. RN counseled her on how to reduce pain and soreness and then left the room so that provider can speak to pt. After about 10 seconds a loud crash was heard. Doctor and RN went into room and found pt unresponsive face down on the floor with arms extended and small jerking movements. Moments after the pt was responsive and able to get on exam table on her own. A&Ox4. She seemed to have hit her head and bit her lip when she fell. After event she was joking about how she sat forward directly after the vaccine to take a picture for social media and that is when she passed out. EMS called and pt transferred to ER for further evaluation.
