Symptom narrative

Pt got up a couple min after vaccination. She went to drop off to get proof printed for school. She had a blank stare then fell, her sister caught her prior to falling but she did hit her head on the side of a pharmacy area. She was out for a couple seconds. Once she came back she slowly sat up. She was given water and popcorn. The ambulance came and took her to the hospital because she was still shaking and dizzy.
