Symptom narrative

Today (7 days after vaccination) mom noticed a flesh-colored welt at the vaccine site on right thigh. Denies previous reaction with last DTaP's. Denies redness. Denies fevers. Denies drainage. Welt is approximately half dollar size. No other vaccinations were given, just the DTaP on 8/1/25.

Current illness

8/3/25: Seen for rash by a virtual provider. Palpable erythema rash w/ lesions scattered across back of shoulders. Few on abdomen and back. 7/24/25: Patient seen at facility with runny nose, cough, decreased appetite X5 days.
