PATIENT RECEIVED AREXVY VACCINATION 08/07/2025 AND IT WAS LATER DISCOVERED THE PATIENT HAD PREVIOUSLY RECEIVED THE SAME VACCINE 10/27/2023. SO THE 2ND SHOT WAS GIVEN OUTSIDE OF CDC GUIDELINES THOUGH NO ACTUAL ADVERSE REACTION HAS BEEN REPORTED BY THE PATIENT WHO IS AWARE OF THE OVERSIGHT.
