The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 27-Jun-2025, the patient was vaccinated with an improperly stored dose of Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) 0.5 mL (strength, frequency, route of administration, and site of administration were not provided) involved lot # was Y019318 which has been verified to be valid; and expiration date was 21-Nov-2026, given as prophylaxis. Temperature excursion was 52.2 degrees Fahrenheit (F), and time frame was unknown by reporter. There was no previous temperature excursion nor report of adverse effect or product quality complaint involved.
