Patient was to recieve meningitis B vaccine (Bexsero), MA/administrator order and administered meningitis A vaccine (MenQuadfi). Patient had already recieved meningitis A (MenQuadfi) vaccine on 7/16/24. Patient, parent and PCP all notified. No adverse reaction noted
