Accidentally pulled proquad instead of MMR. The child received proquad and varicella today, two doses of the chicken pox vaccine. Mother was notified while in the office the moment I realized the error. I explained the error to mother and mother voiced understanding. She is going to monitor the child today and call me if she has concerns. Mother also voiced she will administer Motrin and Tylenol as needed for her child. My manager was notified of the error at the time of the incident as well. I will call the mother later this afternoon, if she does not call me, to follow up and check in on the patient. I plan to do a follow up call tomorrow as well. I advised the mother that I will research the error and call her and notify her of the information that I find.
