Symptom narrative

HPV vaccine was administered. Approximately three minutes post-administration, the 14-year-old patient fainted, striking the back of his head on the counter during fall.

Lab data

On-site provider assessed the 14-year-old patient, who was accompanied by his mother throughout the evaluation. The patient regained consciousness within seconds. Examination revealed no bleeding or hematoma. Pupils were reactive to light. The patient was alert, coherent, stated his name, and demonstrated orientation to place and purpose of the visit.
