PARTIAL DOSE ADMINISTRED (NEEDLE TIP SLIPPED OFF BEFORE VACCINE WAS FULLY INJECTED). FULL DOSE WAS THEN RE-ADMINISTERED PER GLAXOSMITH RECOMMENDATION TO COMPLETED FIRST DOSE.
