Symptom narrative

Patient reported localized injection site reaction including redness, swelling, warmth, tenderness, pain and itching Observation and reassurance provided. Advised application of a clean, cool compress to affected site. Recommended over-the-counter analgesic/antipyretic , acetaminophen or ibuprofen as needed for discomfort. Patient instructed to monitor for worsening symptoms, signs of infection, or systemic reaction and to seek immediate care if these occur.
