On 04-AUG-2025, the patient was vaccinated with an improper stored dose of Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) 0.5 ml (lot #Y013407, which had been verified to be a valid lot number, expiration date reported and confirmed as 01-FEB-2027) as prophylaxis.
