Symptom narrative

Site: Itching at Injection Site-Medium, Site: Redness at Injection Site-Mild, Site: Swelling at Injection Site-Mild, Additional Details: Patient received vaccine on August 3 and started experiencing redness (splotchy rash) and itching around vaccine site and down the side and front of vaccinated arm. States no pain, but is warm to touch and very itchy still. Has not impacted her ability to use the arm or quality of life.
