Patient was here for his 12 month WCC. Seen by Dr and roomed by MA. Patient was due for his 12 month immunizations and MA gave the patient the 6 month ones again. (Patient got Vaxelis-DTaP, IPV, Hib HepB and Rotavirus and PCV 20) (Patient should have gotten Hep A, PCV 20, and MMR/Varicella) Provider was notified. Supervisor and Manager were notified. Incident report was completed and clinical services was notified. Patient will need to come back to get the needed vaccines.
