Symptom narrative

The patient was accidentally given Proquad, MMRV instead of just MMR. The patient was notified of the mistake, the manufacturer was contacted for any possible reactions. Noted that there may be an adverse reaction due to the higher varicella dose. Patient as of now has not reaction. Will contact us if he does have an adverse reaction.
