Symptom narrative

pt reported anxiety before administration. After administration, pt experienced syncope and slumped forward, her head bobbed backwards and then she snapped upright. Pt was not alert for approximately 5 seconds. When patient became alert, she was sweaty and complained of nausea and vomited. Patient remained seated for approximately 15 minutes. Pt stated she was being picked up by a friend and will remain with her for a while.
