Symptom narrative

Pt came in for MMR and Varicella vaccine. Mom requested that they be done separately. When drawing up vaccines, what thought was just MMR was actually MMRV. This was discovered after the vaccinations were given. The MMRV box was inadvertently put in the MMR tray. Pt received both the MMRV and separate Varicella dose of the vaccine.
