Symptom narrative

Patient presented to the clinic for a tetanus shot to meet the university's vaccine requirements. Patient was given Tdap in the left deltoid. We encourage patients to stay in the clinic for 15 minutes for monitoring for any adverse effects, patient went to the first floor main lobby and complained of itching in various parts of her body including her left leg, lower back, and left arm that started approx 5-10 minutes after vaccine administration. Vitals taken and were within normal limits. Patient was evaluated by this RN (who administered the vaccine) and placed on a provider's (APRN) triage spot for evaluation. Patient was discharged home with a diagnosis of pruritius, no anaphylaxis. Patient was instructed by the provider to take Benadryl 25 mg after getting home, and that it was okay to take famotidine 20 mg daily for itching if needed, drink plenty of fluids.
